WAIVER AND RELEASE OF LIABILITY AGREEMENT

This Waiver and Release of Liability Agreement ("Agreement") is executed on this by the undersigned ("Parent/Guardian"), on behalf of their minor child/children ("Participant") listed below, in favor of Creative Class Creative Class Studio, Inc, Inc., its owners, employees, volunteers, agents, affiliates, and representatives (collectively referred to as "Creative Class Studio, Inc").

1. Assumption of Risk
I acknowledge and agree that participation in activities at the Creative Class Creative Class Studio, Inc, Inc. involves inherent risks, including but not limited to:
• Use of art supplies and equipment (e.g., scissors, paints, glue, markers, etc.).
• Physical activities (e.g., crafting, dancing, or other movement-based exercises).
• Interaction with other children and adults.
I voluntarily assume all risks associated with my child’s participation, whether such risks are foreseen or unforeseen, and regardless of whether they arise from negligence or otherwise.

2. Waiver of Liability
To the fullest extent permitted by law, I release, waive, discharge, and hold harmless the Creative Class Studio, Inc from any and all liability, claims, demands, damages, costs, or causes of action arising out of my child’s participation in activities at the Creative Class Studio, Inc, including those caused by negligence.

3. Indemnification
I agree to indemnify and hold harmless the Creative Class Creative Class Studio, Inc Inc. from any claims, damages, or expenses arising from my child’s participation, including attorney fees and costs, incurred as a result of claims brought by third parties.

4. Medical Emergencies
In the event of a medical emergency involving my child, I authorize the Creative Class Studio, Inc to seek emergency medical treatment and care on behalf of my child. I agree to bear all costs associated with such treatment and will not hold the Creative Class Studio, Inc liable for any medical expenses or outcomes.

5. Supervision by Parents/Guardians
I understand and agree that parents or guardians are required to supervise their child/children at all times during their participation in Creative Class Studio, Inc activities. The Creative Class Studio, Inc is not responsible for childcare or unsupervised children. Parents/Guardians must remain on the premises and actively supervise their child’s participation to ensure their safety.

8. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the Province of Ontario, without regard to conflict of law principles.

9. Acknowledgment of Understanding
By signing below, I certify that I have read and understand this Agreement. I acknowledge that I am voluntarily signing this document and agree to its terms on behalf of my minor child/children.
